Wait I think maybe that in the past live it's a one-sided love. The dragon loves his caretaker and he loves him back but not sexually then in the next life it's a reciprocated thing. In this story from what I remember omegas are women and they even say it in the first chapter and it’s unusual to have a male omega and omegas are described as pretty and frail creatures kinda just there to give birth like in a lot of omegaverse.
