I believe it's based on the number of topics (or comments) that ppl have made on the manga
'popularity' bases on rating & votes.
If you care only rating, you'll get many almost 10 with few votes which's unreliable.

This one is my favourite!!! It makes me sad, angry, and laughed hard. It's a masterpiece I would say.
I only remember this small part from the extra. Both of them are in a small prison. The uke want to use the toilet to take a dump. But he's too embarrassed because it's an open toilet. The seme noticed then he made a fuss to get the guards attention, pretending he has phobia being inside a small room, then the guards moved him to another prison so that the uke could use the toilet with ease.
