Umm it usually means a feminine boy look. Or a guy who looks 12 (skinny and lean and short). At least from my experience, it's not a gay slur but can be used negatively
The t slur is something people use against trans people (Tr4nny). However twinks on the other hand is what you call a guy that has very feminine features
