The intro leaves a strong impression that they are not together. If Hikaru were continuing to rape Haruki for that long he wouldn't be hurt, he'd be happy like he was in this chapter.

That meant nothing. The mangaka actually said something like "has it become brother BL?" It was presented as a question so people would purchase/read the chapter to find out the answer. Authors say stuff like that all the time to advertise and increase interest for a new release. It would be absurd to give away such a huge plot development.
