I think they’re the doctors at the clinic that the older brother supposed to have pancreas implanting. On the surface they advertise that they would use 3D printing technology to create the organs but in fact they just replicate the patients and treated the replicated ones as a source of organs replacement.
My thoughts is that may be they’ve got a tracking device set on the real big brother, so they’re the first to notice his death, afraid to lose the money they invested on the dummy, they thought about letting him replace the real older brother, that way they can get the hospital expense for both the implants and for treating his wounds from the accident
