so basically Kuze and Taichi broke up and then a few years later Touma made Kuze, Taichi, Masumi and him meet up all together again after a long time and by then Kuze had already found someone she wanted to marry same as Masumi. The last chapter basically is from Touma's perspective and is implying that Touma and Taichi got together as at the beginning of the reception of Kuze's wedding Touma wrote his last name as "Ichinose" the same last name of Taichi's like if they got married. Later on Touma and Taichi meet together after coming from the weddings, since Taichi's other high school friends got married in the same day, and they share the picture's they took. Then they have a conversation regarding the concept "would you choose your lover or your best friend" which brings back memories for them from high school. Then later on as Taichi reaches out for Touma's hand you can see there is a engagement ring and that the hand Taichi holds is bigger than his confirming that it's Touma lol.
