For the last time, if you read the manga, Mizuki does not fall for Kotarou until Kotarou became an adult. Also, Mizuki was also a minor when they met. You can stay the gap is problematic because Kotarou did get married at 21 and Mizuki at 33 but Mizuki did not have any sexual intentions when Kotarou was a child and treated him like an adored little brother.
