I have read hundreds of isekai manga/novel. And mynoghra can amazed & hooked me on the 3rd chapter. The story has so much potential. There are only a couple of isekai title that can pushed me to machine translate the webnovel asap, and this is one of those title.
I would focus more on the phycopath aspect than the sex (●￣(ｴ)￣●)... its been truly a long time since i really “read” a yaoi manga... usually i just skimming the pictures over to the sex part... author, i’m satisfied... ლ(´ڡ`ლ) thank you so much for this great work!

It is a happy end.
The crux of the matter that is causing suffering for both of them is his inability to forgive himself and hers to forgive both him and herself. Once this was solved they could be happy.
And she forgives him very soon in the novel (in fact, it will happen in the next chapter of the manga) because she actually always knew that what he did had to be done and that he did it in the best possible way at the time. She also saw that he had always been blaming himself and was never able to feel at peace so she wanted to free him of the guilt.
Now for her own feelings of guilty it was a bit more triky because she couldn't forgive herself for both still being alive while her whole family died and for being anaware of all the terrible things they did while living in blessing for years. So she felt she had failed everyone; the people of the fief, her loved ones, and even the new lord that had to go through a hard time for many years because of his penance towards her.
Fortunately, some problems will happen in the next chapters that will force them to face each other, talk, and finally let go of the past and the weigh on their hearts. They will both finally forgive themselves enough to allow themselves to live hapilly from them on.
She will also discover that he had actually always truly loved her from the time they spent toguether and all along his years alone untill she appeared again. So what he felt for her wasn't only guilt but also love, thought he didn't think he was worthy of having these feelings for the person he betried.
In a nut shell both had always loved each other but could't let go of all the emotional bagage of their previous interaction. They didn't think they were worthy or right for the other untill being forced by the circunstancies to acknowlege that they couldn't be happy without each other.
At the end they will get married and live in happines and peace. Also, the servants of the mansion that had always loved and respected her in her previous life will discover at the end that she was the reincarnation of their lady and give her a warm reception, being glad she came back.
