fr though i think the author is being rushed, like most of these pro artists are now, and they did not consult their character specs. this is like a 157cm twink next to a 2m hunk this stuff used to be so common, even when characters were supposed to be like 170cm and 175cm, artists used to draw them like this.

i think the main story was fine. i enjoyed the inversion of expectation, with the desperate softboy being the top and the gruff alt guy being the bottom.
i think people are allowed to be in relationships primarily bc the sex is good, if that's what they've communicated between themselves.
but i also think the side story added a little more, and it makes it pretty clear that the college guy genuinely likes the older guy.
my initial rating was 2/5, but i think, rereading other comics and cooking back to this, i think it's more a 4/5.

why that dick ugly? it's a body mod called pearling.
basically he got implants. it's done at body mod shops like extreme piercing parlors and the artist implants titanium, silicone, or whatever material balls under the skin. it's meant to improve the pleasure of the partner(s). sometimes it doesn't heal well and there's a chance of rejection and infection. personally, i wouldn't do it, but it's pretty similar to people getting titanium balls implanted on their foreheads to resemble horns or alien growths. titanium is porous, so bodies hold onto them pretty well. also generally you don't get mods like that done like you might a tattoo or ear piercing. few artists will perform the mod, and it requires an appointment in advance (how long, i do not know).
if you think the ugly pp is viscerally upsetting, please don't look up any genital body mods, there's a lot more extreme stuff and a lot of surgical progress photos mixed in.
i'm sure this has been mentioned a million times before but in my country, an amber alert is a mass text + alert tone notifying people in the area of a reported missing/kidnapped child so this title hurts my brain
