Could that mean that his sister was an illegitimate child and the fortune teller said the mother was praying for him when the mother didn't and we could see in the previous chapter of the flashback where the mom put a knife to his neck!!!!! unless they were twins and the father got rid of the daughter so there's no fighting over the throne maybe and idk if females could rule during that time but it's just an assumption as to why he would get rid of her. I think I got it, I don't think his mother and father loved each other because it was maybe an arranged marriage??? So what if the mother had a previous lover and was force to marry the emperor and by than she was already pregnant with her lovers baby and later the emperor found out and got rid of the child and was force to give birth to the duke and that's why he hated him so much!!!!!!
