In the previous page, the scene focuses on Karasuma watching Takaba happily observing his nephews, this makes Karasuma realize that Takaba naturally loves children, that he is so good with them, making it very likely Takaba wants children of his own and that he would be a great father. That scene saddens Karasuma because it seems that he can't have children, or at least, he has not been able to get pregnant, which is why he apologizes to Takaba. He feels guilty for not being able to give him children.
Many of us speculate that Karasuma is having difficulty getting pregnant because of his overuse of the heat suppressants because of his previous hypersexual lifestyle, but I would like to think that since he's no longer abusing his body like before, the Mangaka may go the happy route and surprise us with a baby :)
I really hope so! I remember the first months trying to get pregnant were so sad and depressing T_T
But I could do it by the third month, so I really hope his body goes back to normal soon and they came be a happy family <3

It is a catch 22, because part of the inability to get pregnant is psychological, the mental stress that is exacerbated by the fact that you are not getting pregnant, which makes it even harder to conceive because of the mental stress! which may be an element in Karasuma's case. That is why it's common for couples that give up and decide to adopt, to conceive soon after. Once the mental stress is removed, conception happens. Just shows the power of the mind!
