He’s not judging it’s not really natural in that time, due to the type of food, and if he knows that she didn’t look like that from the story he would be concerned.
Also in that time she would be shamed for having that much weight on since they were critical about womens figures, so him being concerned would help her solve what cause the weight gain. She might be ill.

I'm gonna guess it's bc she learned how to hunt after meeting him, and when she talked to him it sounded like she was being neglected/underfed in her family

Yeah the girl already was really dependent on her servants growing up cause of her loneliness but the servant is the one that continued manipulating her and was also the one that caused her weight gain, if you looked at the flashbacks. The servant was also the one that would torture the people back then and it said that the original girl couldn’t even move due to the weight gain, so maybe she was just an unwilling bystander and the servant was just controlling the situation
