I think in real life men's nipples are sensitive, prone to tickles mostly but can also be pleasured through them. As for yaoi I guess its just another stereotype that nipple work is for ukes.
As for seme not being fully naked, the things is mentioned in the seme/uke handbook that semes hardly get naked completely and they just remove their shirts and being completely naked is a uke thing.
