the symbolism behind the story

The way I see it, this oneshot is like an allegory. My interpretation is that it's about a couple, being together quite some time, and one of them is suffering from an illness, probably a mental issue, pictured here as a third arm. Something like Kafka's "Metamorphosis".
His lover, very much in love with him, decided to stay with him, to support him, protect him and in the end save him. With his help, slowly will get better (at some point, at page 8, we learn that the life circle of the arm becomes shorter) and in the end be completely cured.
But, like every treatment, it takes time and patience to win the disease, so what we see at this manga it's just one day during the process of the battle.
Well that's just an idea though (⌒▽⌒)

    Good perspective. I saw it as how the hardships become "just another regular day" for people who live together. Physical illness, mental illness, or whatever, when you live someone, you deal and it becomes "the usual."
