Well since the shinigami/death gods are "born" from humans who committed suicide, undertaker could have been his first lover(oh how i wish!!), could have been really great and close friends ooor the are blood related Dun dun duuun (wouldn't that be epic? Undertaker is Ciel's uncle XD ヾ(☆▽☆) )
