The Doc managed to get the bullet out of Cain's heart but also resulted in the MC, Gyeong (?) Almost dying. Cain asked the Doc for a favour to bring back Gyeong to life. And I think Cain would have to go back to Hell maybe for the war. That's all I've understood.
