Because he was born with male organs including a fallopian tube and uterus. It is actually a legit condition that a person can be born with. That's why his father said that he is not a man and also not a woman and that is disgusting and hw was extremely hurt. You can just google it .maybe it's called "intersex"
He's intersex. However, with his type, it's biologically impossible for this to happen. So just use your imagination.

Yes, it's not the same in all cases, but it's always genetic. Your genes affect your phenotype. You can be visibly intersex or nonvisibly intersex. I never said it's biologically impossible to be intersex like the character in the book; just to have children in the circumstances he is implied to be in.
Seeing as his parents knew he's intersex but he didn't, it's highly likely that he was born with both a penis and vagina, then his parents had him get surgery to close up the vagina (though it's odd they wouldn't remove the uterus as well, which is weird). Another alternative is that he had an x-ray at a young age that showed he had a uterus and ovaries.
The only way he and the other guy in this story could have no clue he's intersex is if he only has a penis. Meaning that the implication here is basically that they had anal and somehow the sperm got up in there and made him pregnant.
Intersex is 100% possible. Getting pregnant in his implied circumstances isn't.
