    *************SPOILERS**********SPOILERS*************SPOILERS*********

From what I gathered, when the cat took Doujin away from Yellow Dragon after they both were stabbed, the cat took him to his world (real world). However, I think it's because of Doujin's strong connection with the Yellow Dragon that allowed him to 1) still remember him even though he is back in the real world and 2) find the tea shop in the end. If you treat the tea shop as a "portal to the spirit realm" and Kingdom of Yooguk as the actual spirit kingdom, then it makes more sense. It can also explain why Doujin was able to hear the cat but not see the cat when following the meows to the tea shop because the cat is a spirit animal.
Once in the tea shop/portal (or bridge to spirit world), the goddess of creation called all of the spirit animals involved basically there to judge him. The spirit animals spoke fondly of him thus allowing the goddess of creation to grant Doujin one single wish. He wished to go where the sleeping dragon was and so he went. Now..... how in the hell the 26th version of Doujin got the pearl (the power source of the gods) I have no clue. BUT, going back to Doujin's strong connection to the spirit realm can explain how he was able to retrieve it in the dream. So once Doujin gave Yellow Dragon the pearl, the Dragon gained all of his power back and regained his sanity. Then they had a heart to heart talk over everything that happened.
Yellow Dragon, during the discussion with Doujin, was reflecting that basically all/most of everything bad that happened in this story was caused by his greed so now he must confront the goddess of creation and repent. Now what was his punishment? Probably him having to be without Doujin for x amount of years. In that time, Doujin would go back to his world and live with the ones that he love with the promise (ring) that one day he will return to Yellow Dragon. When the niece was still young, she told Doujin that she wants to meet the mysterious lover man so that's why the author included her in the final dream where she sees her young uncle and the Yellow Dragon together at last happy and both of them walking to the spirit realm. The last page showing Doujin dead in the hospital reinforces the idea that (in the story) when people die (or go into a comma like Doujin did in the beginning), they enter the spirit realm and reside in one of the 5 kingdoms.
One of the many things that I found was super adorable was the part in the end where Yellow Dragon and Doujin was having the discussion in the sealed urn. Yellow Dragon mentions that the cat was a guide for Doujin so that he wouldn't loose his way and now he (Dragon) must be a guide for Doujin. I think this was foreshadowing the ending where when Doujin dies, Yellow Dragon would meet and guide him to the spirit realm.
