Yea it may seem a little confusing. But the vampires choose who they want to give their heart to. Hisato said that he would know who to give his heart to just by seeing them. Maybe it's something to do with feeling or instinct. Once the vampire chooses the person they want to give their heart to, they can't take it back and cannot change the person. They have to die with them. I hope that you understood some of it. This is all from my understanding.
