Hallucinations.
Getting beaten up and see someone dying causes people fear also known as phobia. From shock and terror at a young age MC develops schizophrenia.

this has been bothering me while im reading this like how can he see monsters on people or how can he see his childhood self, like how?! he sees monsters on peoples face. can someone explain?
