Like idk about the period blood or the pregnant fetish ones, but BDSM can go a lot more extreme then just strangulation to unconsciousness, but still be safe for the parties involved. Usually there is a lot more communication and clarifications. This is also a really limited view on what it means to dominate and submit, its not as shallow as just dealing and handling pain.
