You should go and check out more yaoi mangas. Because this thing actually does exist. Go and check out some of minase masara senseis works. I know at least one where it happens.

I don't know why anyone ever even discusses who will end up with who. Obviously the two people on the front cover will end up being the main couple, at least by the end. I've never ever read a manga where the final pairing wasn't the couple on the cover. Pretty sure that sort of thing doesn't exist...
