I'm not sure how it started with one of the students becoming a target of a possible traitor. But there's a chapter that has Aizawa-sensei talking to the education board and he mentioned that there was a possibility of a traitor within the group.

When the LOV kept showing up where they were (even during super secret away training). The faculty were trying to figure out how/why they were able to track them and someone (I think it was Present Mic...but not sure) suggested there could be a traitor. Other options were discussed too.
