I don't understand the story anymore, im just watching the drawing (〜￣△￣)〜

What's hard to follow? Cain has an injury and he needs the doctor to fix it, Sid is betraying him and doesn't want him better... That's basically what's happening. And there's still a war between Satan and Cain going on
