I'm pretty sure it's not homophobia, I think it's because they're blood-related--which is rather uncomfortable to think about. Sakura's mom and Tomoyo'a mom are cousins, which makes the two related. This entire manga has...questionable choices in a lot of the relationships, (- v -).
