From page 24 to 25, Hifumi asks the questions of would Doppo actually take care of himself if he did present as Omega and Doppo answers "no" to all of them.
Also, if Doppo had to of presented as an omega he also thinks he shouldn't be by Hifumi's side anymore, thinking it may cause him unwanted trouble.
Hifumi probably has some obsessive infatuation with Doppo and didn't want him leaving over the blood work so he switched it, assuming Doppo would be an omega, which was spot on.
