ack!! please help!!

ok, so i've spent the last hour digging through the yaoi manga directory trying to spark my memories on this manga, and i still can't find it ┗( T﹏T )┛ i give up... i need some help guys/gals/pals:

it's a yaoi about these two guys(duh, sorry that's dumb to say, but whatever XD).... i think i remember that the author had this unique thing going on where their ages were labelled throughout the story--if that's helpful. so anyway, after they got together(ugh, sorry i can't remember most of the early plot....), something happened, and they broke up--i think they just started drifting apart, but i'm not sure. anyhoo, five years later or so, they decide to get back together, then they're lives go on together and they grow old together--i'm pretty sure they got married and got children/grandchildren. one of them died in their sixties or so i think---i remember that he was in the hospital and asked the other to buy him some apples or something, and while the other was gone, he ended up dying. it was mega sad, but anyway, the other guy lived on until his eighties or so and after he died, it showed them meeting again in heaven happily. the whole thing was one big emotional rollercoaster, and i cried so much--i don't usually cry when i read, but this one really got me...
