Idk for sure but since I know it somewhat sex related I'm assuming some very kinky sex like they treat him as toy and do whatever they want with him to the point that its unbearable since he did trow up at one point

i think its also drugs idk because i showed twice that afterwards he gets nosebleeds. It could be from overexertion on the body but kinda feel like he is being drugged and they do stuff to him.
