Maybe if this was a shoujo or Josei manga but most yaoi with incest themes generally go all out and do things in full mesure with actual blood relations.

No need to get all smartass on me. Of course there’ll be a couple that aren’t twisted. That’s why I said usually. With the amount of mangas being pumped out each month, there’s no way all of them will be twisted.
