İ see what you mean but dying while giving birth and dying because of poison is totally different thing. Of course he can still gone crazy but i believe her death is kind of related to her pregnancy as well

according to a wmmap novel reader from tumblr: in the novel, it was stated that anastacius and penelope bribed maids to poison on diana's drink so she'd die after giving birth to athy (idk if this is real but if it is, the ruby palace massacre would make so much sense)
