Can someone reexplain to me the 2 story ? I'm so confused 😭

Im not sure but it seems like the seme on the second story was so into the character he was portraying which is "sungwoo" to the point that even outside the set, he acted as sungwoo. Hence, the doctor saying " confusion of reality" . so like their relationship is basically a "pretend" because after the hospital visit the seme seems to have snap out of it and acted the same way he did before all this acting happened . ( thats just my guess though, its not accurate)
