I think the author is trying to depict how far the Uke went with depriving himself or a healthy lifestyle due to his family abandonment. He’s extremely nonchalant about his well being, the guy ONLY eats bread! Clearly his malnutrition has led to stunt growth. Google and look it up, it’s a real thing. Usually with abusive, mentally ill and tragedy stories the characters will be depicted frail, sickly and with malnutrition because abuse, mental illness and trauma are the leading cause for eating disorders. Kudos to the author for a realist depiction of the Uke given his mental state and traumatic past!

We have a vast number of gay men that act with “gayish mannerisms”, you will most certainly be able to tell that they are gay by the way they look, talk, dress, by their gestures and walk. A lot of gay people I follow and my gay friends are like this and there’s always “people like you” who comment or attack them saying things “Why don’t you be more manly”, “Real men don’t look like that”, “So you want to be a girl”, “Do you even have a dick between those legs”! Leave your expectations of how “real men” should be to yourself. True!! One of my work mates at my last job was pretty small and skinny and everyone would always pass those “you’re too skinny for a man” “go to the gym, how can a man be so small” remarks. First of all , stop putting words in my mouth. Secondly i have not said anything offensive toward feminine gay people . My problem is that the authors characterise ukes as the feminine gays , as if masculine male wouldn't bottom. My main problem is with the age, I don't want to look at sex between an underage child and an adult , i don't have a problem with twinks, for example i love bl hotel, the uke is feminine and skinny but doesn't look too young to be having sex.
