IMO, he's called half-omega, partly because of his physical characteristics, but it's also because his physiology and sexual maturity is different from what's considered "normal" for omegas. He didn't experience his heat during the expected age range, his heat is highly irregular and his omega pheromones are stronger. He also doesn't respond properly to omega medication, which is why the doctor had to resort to the rather extreme treatment of "training" Doyun with an alpha.
Maybe it's because of some versions of omegaverse where there are dominant and recessive omegas and alphas? Or maybe he's a sub-omega with a half omegas Gene and half something else.

I wonder why doyun called as "half-omega" not "omega".
Is it only because his physical that doesn't show him as an omega? Or is there anything else? The doctor once said doyun is quite special
