Not sure why she can only use it 3 times, but as to why she was trapped. Because she tried to go back in time and that failed, seems like her orgiianl body got destroyed but the lamp is a magical equipment and so it absorbed her soul

Im pretty sure she said that she only has enough mana for 3 wishes. So the amount of wishes depends on her mana reserve. What she does most of the time is changing the strings of fate type shit (sometimes even more simple than that) but since she is someone who defies the laws of nature, shes probably limited. That was just my take on it tho.
