It's not a simple fetish, it's a crippling fetish. He was straight up objectifying the hands and not giving a shit about the humans they were attached to. He has already been undergoing treatment and getting better before the start of the story, which is why it's toned down but his psychiatrist still afraid not him derailing.
Also, add the fact that he is a public figure in a conservative country and such scandal could not only destroy his own career and reputation, but also the career and reputations of the other famous celebrities he dated.
i mean a fetish is a fetish at most but they treat him like he's diseased or something, like did he really need to get treatment for it???
