I would expect Lee to lie to him while he is awake...what would be the point in lying while he is unconscious unless it is the truth. And by truth I think Lee's I love you means I like your body, I like having sex with you and the way you talk to me gets me off...rather than the standard meaning...ie how normal folks with equals say I love u to each other.
At this point it isnt love yet, but definitely there is something going on in gyung's mind considering how he was taken aback by hyungdo's confession. Lets just wait a bit for more developments.
