It's all real. Dojin really does have a sister and an adorable little niece. The Yellow Dragon is also real. Dojin and The Yellow Dragon are from two different worlds. When Dojin got poisoned, The Yellow Dragon showed him a dream that was based on his home-world. That's why Dojin was able to pick up on small differences, like the fact that his sister in the dream was cooking with cucumber when in reality his sister does not eat cucumber.
