1) Yes just like @春日 said, you can see Myaa-chan shipping KagiHira in the original story and if I remember right Kagiura once appeared in it.
2) We're in the "past" here, we're around 1 year before the current event of Sasaki to Miyano, we're at the same time as SasaMyaa vol1, when they barely knew eachother and weren't aware of their feelings yet (even if Sasaki was interested).
It might be why Miyano don't seem to really know about Kagiura unlike in the main one and the fact that Sasaki wasn't whining (haha) about leaving Myaa-chan for the trip.
The scanlators said they'll add the official timeline in the extra at the end of this volume.
3) They're the same, they're both first years.
Same universe. Kagi and Miyano are the same age.
Wait a little longer and a timeline will be published. The author included one with this book to clear up confusion.
For clarity though: this volume is set around Volume 1/2 of SasaMiya.

Miyano is indeed a second year but only in the current event.
In the first and second volume of Sasaki to Miyano he is a first year (they had a first cultural festival where M's class had a maid café while S's was a haunted house. They then had exams, a closing ceremony and went up a grade at the end of volume 2 (around chapter 10) That's when Miyano became a 2nd year and Sasaki/Hirano 3rd years.
And since it was confirmed that volume 1 of each book have the same timeline then they're both first years in the current chapters of this story ٩(๑❛ᴗ❛๑)۶
