Yes, they are both alphas, but the doctor does say in one of the later chapters (I believe it's chapter 15) that's he's not into omegas. He has a thing for alphas, and most people believe he likes the brother.

So people are shipping the brother and the doctor but see aren’t they both alphas well it’s not certain the brother is but we know the doctor is. And from reading the destiny omegaverse manga that had an alpha and beta together which broke my heart already. If the two of them are alphas i can’t imagine the possibilities it will do to my heart again.
