She's dying, and she knows it.

There's a reason why she doesn't have any thought bubbles in the last chapter. It's cause it would reveal that she is the only one who needs to know when she'll die and her condition.

She doesn't want her friends to worry again and she wants to fill her bucket list.

You can't change your fate. That's what the story implied.
