In Japan, BL means both yaoi and shounen-ai. Any manga with two main male characters who fall in love with each other is referred to as BL. When I went to Japan to meet my cousin, she said no one actually says "yaoi" or "shounen ai" so it's more of a "west thing" she said. The normal term is BL because shounen ai is literally japanese for Boy (shounen) Love (Ai).
But outside Japan where Yaoi and Shounen Ai aren't used interchangeably, Yaoi is more hardcore and Shounen Ai more fluffy. But really, they're both categorized under BL.
Doujinshi is not a subset of BL, it's just fanmade or self-published work. Just like fanfiction or fanart. Doujinshi doesn't necessarily have to be about a couple and romance. It can also be about one person. Maybe you've only read BL-type doujinshi?
