Through the years 1980/1990 whilst the 'Act Up-Movements', the activsts starting to use that word by themselve and reclaiming it with a benefical and political meaning. Nowadays it's used by all kinds of Groups: gays, lesbian, bisexuals, intersexual, transgender, pansexuellen, asexuellen, BDSMlern and also polyamory. In the nutshell people which see themselve beside the so called norm. Whereas gay just refers to homosexual men.
Lesbians use gay to describe themselves, too, conversationally, so I'd say real-world usage would define gay for homosexuality rather than exclusive for homosexual men.
