It was because he didn't treat himself/wasn't treated well that he gave off such a first impression. Koogi's style also changed over the (almost) 2 years.
Sangwoo for example also looks different than he does in the earlier chapters, more bulkier now while Bum is more thicc and not getting abused anymore which could be also a reason for his slightly healthier appearance. Still wish he got to eat more, but I guess he's okay with this since he's used to being undernourished.

Yes, that's well reasoned. However if Koogi wants to show that Bum is healthier now than before that seems ominous for Bum, nothing good ever comes without something terrible. The implications for the deliberateness of the subtle changes really give me chills.. ۶۶(⊙…⊙)۶۶
