They are actually men, prisoners just call them "ladies" etc. because they're feminine. And when you're for way too long in a prison and deprived of sex, you start treating other men like a possible sex object even if you're straight.

I'm still at chapter 3 so I don't know if it's explained later in the story, but how come there are women in the same prison? o.O Because there are some prisons whith both male and female, but they don't share common areas ┗( T﹏T )┛
