Knotting is when the penis becomes bigger inside to keep the sperm in, to increase chances of pregnancy. Having anal sex hurts as is, but if the penis enlarges inside it's going to hurt even more.

It's at the base of the penis, where it connects with the balls and swells to ensure all sperm is used and as a protection thing.

The seme is still an asshole, taking any rights away from the uke. And if dogs are anything to go by, when tied, or "knotted" as used here, it can be very painful for a bitch so no wonder he's in pain. And even though the uke was a gold digger at the beginning that changed, because if he still was then he'd be over the moon with the possessiveness of the seme.
