For instance, Seno actions are way more debasing and indicative of low self esteem because he knows he is being used as replacement, he is the one getting humiliating and cold treatment and still he goes even further in degrading himself by pretending to be somebody else just for crumbs. I imagine what kind of traumatic/story he has that has made him to feel so unworthy of respect to the point he submits himself to be treated like that and stay. He telling Natsuka to go to Hiro was maybe his only moment of pride so I'm not sure what it is about trust or not.
Natsuka and Hiro dynamic is completely different. Hiro is actually nice to Natsuka and treats him well. Natsuka seems to be going through a regular unrequited love story where the subject of our desire is unaware of our feelings but it's not like he is putting himself through humiliation and degradation and he doesn't seem the type who suffers of low self esteem.
Seno is clearly someone with a mental problem because no one should accept being treated like that and still stay. Natsuka is an asshole not because he is using the other as fuck buddy --- as you said, he was clear it was about sex but it's his whole disregard for someone else that makes him an asshole. He ignores Seno in the hospital, he treats him badly, he knows he just came from the hospital and let him sleep in the floor, and, as much as Seno knew he wasn't interested in him, the fact that Natsuka talks so coldly about someone else he is intimated with makes him very cruel as a person. You don't need to be in love with someone to be nice or have some compassion or be a little polite. Just like Hiro is not in love with Natsuka but treats him with respect and cordially. Look how Natsuka treats Seno -- like he is kicking a dog.
