I believe that the kid’s ribs broke/got a fracture due to the shovel trauma. That could be a passage way for the cardiac massage
are skin usually that thin to rip open and then being able to take grasp of the heart?
Once there’s already a break and the skin, it’s relatively easy to rip it with enough force. It’ll still be a struggle though, I guess.
Let’s just assume that the puncture made by the shovel was big enough to let his hand in lol it’s still a work of fiction after all. I mean. Who has that good a hearing ability? Haha

not a medical student but pretty sure u can't just get a hold of the heart by shoving ur hand into an open wound like that, where did the kids ribs go?!?
But still kinda funny and weird all in one so can't wait for the rest :)
