I don't think BJ MD is Alex's first love interest. BJ MD is just there to add some drama... I don't mind though since BJ MD looks full of swag and he's also hot, lol. I would have preferred a really polite AND hot seme to have come into this story, but I believe this artist just wants a bunch of assholes to be around DG. I think the only non-asshole characters are DG's supportive coworkers! I love Chanwoo! (=・ω・=)

So we're introduced to BJ MD (#2 on the BJ ranking we see in chapter 7) and I enjoyed his character for about 2 seconds until it was revealed he's just a sly version of Alex. Poor DG... He's totally going to get all caught up in this.
I am still waiting for more of Alex's backstory. Like, why do BJ MD and BJ Alex know each other's real identities? Why is Alex so angry/worried? My thoughts are he doesn't want to lose his top spot and perhaps the first love interest Alex had may still play a role here and BJ MD will bring him back in.
Or... BJ MD is going to ask DG to do SM play on his stream and that will make Alex go nuts. I kind of want to see that!
