tis a pun my friend. "sweet" can mean nice but can also mean the flavour sweet which is often the result of sugar which can lead to diabetes.

Well technically from a science perspective, the ineffeciency of the pancreas in the production of insulin for regulating blood sugar levels, causes diabetes. So my comment in reality, is invalid!!
