Alpha have the highest semen count and the chance of becoming impregnated by the Alpha during heat is 100%. However, this is mostly possible if the Alpha's partner is an omega. Betas can be impregnated by Alpha however this is extremely difficult. Since Betas doesn't have a high fertility rate (compare to omegas who have the lowest), Betas sometimes have a hard time becoming pregnant.
Miyauchi possibly thought at some point during their relationship, he and Jirou may start a family in the near future like their masters. However, since Betas have a hard time becoming pregnant by their Alpha partner; Miyauchi went to a fertility center for some help. Unfortunately, the doctor delivered Miyauchi devastating news. The news devastated Miyauchi as he believed that he won't be able to grant Jirou happiness.

Agree! Everyone can get pregnant even alphas and betas. It's just that, due to hereditary reasons or maybe some trauma due to child abuse in the past (says the doctor), Miyauchi became infertile and therefore can't give Kudou "happiness".
He's not infertile but near it. Like the doc said, it's very "difficult" to get pregnant which is pretty much like a 1% chamce.
